The Uniform Civil Procedure Rules,outlines forms to be used in the Three Court Jurisdictions in Queensland. The forms used are common to all Three Jurisdictions,except The Minor Debt Claim, Form 3 ,which is used solely in the Magistrates Court.
When using any of the listed forms, they should be completed ,taking particular notice of the general instructions

1. The number and name at the top of each form, and the endnotes to a form, are not part of the form itself. When using a form set out the form number and the relevant rule number at the lower left corner of the first page.
2. Words in italics are instructions for the completion of the form.
3. Words in square brackets are to be inserted or omitted as is appropriate in the particular case.
   For proceedings in the District Court or Magistrates Courts, change Court name accordingly.
   For proceedings commenced by Originating Application, for "plaintiff" substitute "applicant".
   For proceedings commenced by Originating Application, for "defendant" substitute "respondent".
